    The Care Coordinator assists the Registered Dental Hygienist (RDH) in many aspects of patient care and treatment in community settings such as schools and preschools.

    • Assist as needed with patient care activities to include scheduling, confirmation telephone calls, retrieving/dispensing electronic charts, data input, arranging and following up on referrals, and providing oral health education as directed by the RDH
    • Assist with explaining to patients, parents, caregivers and agency staff, the virtual Dental Home System of Care policies/procedures as realted to dental treatment, and research
    • Assist clients to enroll in oral health care coverage or insurance
    • Assist clients to register for program services including obtaining consent
    • Assist with collecting and recording electronic data
    • Assist with taking and labeling dental radiographs and intraoral photos
    • Arrange for follow-up care with participating dentists and document referral follow-up activities
    • Report supply needs and restock supplies in a timely manner according to established procedures
    • Work with the RDH to maintain dental equipment and comply with OSHA and infection control standards
    • Provide patient coordination including parent follow up calls
    • Attend and participate in staff meetings and other staff events
    • Particpate in quality improvement activities
    • Provide outcome data for a and meet periodically with personnel from site and additional evaluators
    • Perform allother duties as assigned



    • High School Diploma
    • Dental Assistant Certificate
    • Bilingual English/Spanish


    • RDA Certificate

    Required Skills/Knowledge/Abilities

    • Ability to work in a fast paced environment and maintain professional composure and demeanor
    • Basic knowledge of computers, internet use, MS Office and data entry skills
    • Ability to manage multiple priorities and projects simultaneously
    • Knowledge about enrolling patients in health plans in the county preferred, but not required
    • Ability to complete work assigned with limited supervision
    • Ablity to communicate effectively with staff and patients
    • Ability to write accurate and clear documentation
    • Ability to apply patient management and time management skills
