Responsibilities

Provide care management services to the parents or guardians of patients who are high risk for health deterioration or poor health outcomes. Work towards improving health outcomes through scheduling preventive and maintenance care appointments, educating patients, building a strong relationship between patients and clinicians. Enhance communication and continuity of care using a culturally-sensitive approach with all patients whether by person, by telephone, via text message, or through the VCC Patient Portal. Work towards increasing patient access to appointments, services and health care resources to support patients’ self-care management goals. Provide teen services support in family planning, birth control methods and sexual health education.

  • Provide coordinated team care that addresses current health issues, preventive care and health maintenance follow-up to improve health outcomes and reduce health disparities
  • Provide end of visit interviews, initiate patient engagement conversations as directed by the Clinicians, documenting in the patients’ progress note or Care Plan
  • Coach parents/guardians of patients, using positive reinforcement, encouragement and a flexible approach to address critical issues to help patients develop achievable self-management care plan goals, presenting new skills using a step-by-step process
  • Act as point of contact for parents/guardians and family members of patients asking questions or raising concerns through the patient portal, or via telephone and/or texting
  • Use motivational interviewing to gather patient information and understand barriers from the patient’s view to enhance efforts to engage, educate and empower behavior changes
  • Reinforce information given to the patient and/or family with handouts to improve patient self-management skills and communication
  • Provide Asthma education to parents and patient; demonstrate the use of the spacer and review medication; assist in the completion of the asthma action plan; communicate directly with parents regarding meds, refills and to schedule appointments; perform spirometry testing and document into NextGen using care coordination template
  • Provide family planning/sexual health education resources during teen visits; screen for STI and pregnancy; perform screener visits for birth control and schedule appointments for LARCS; provide condoms if requested; accurately document workflow on family planning template

Qualifications

Minimum

  • Bachelor’s degree in health-related field or equivalent combination of education and experience
  • Certification/minimum two years’ education in chronic disease/health
  • Bilingual English/Spanish

Preferred

  • Bachelor’s degree in Sociology, Social Work or Health Education
  • Minimum two years’ experience providing care coordination services

Required Skills/Knowledge/Abilities

  • Ability and willingness to work flexible hours, including evenings and weekends
  • Excellent verbal and written skills necessary for communication with patients/clients, providers and other staff
  • Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs
  • Ability to work independently, take initiative and be proactive
  • Ability to analyze, prioritize and meet deadlines
  • Experience/familiarity with computers and proficient in Microsoft Office products, specifically Word and Excel

Pay Range

$20.00 - $24.00
