Manage a caseload of 150-200 assigned patients throughout pregnancy and post partum; collaborate with other Care Team Members to address patient needs and concerns, and encourage attendance at all appointments with a focus optimizing health outcomes. Deliver VCC’s Comprehensive Perinatal Service Program (CPSP) by delivering health education, providing case coordination, completing comprehensive assessments, coordinating support service and monitoring nutrition, education and psychosocial interventions and referrals.

  • Provide tailored health education to patients on a wide variety of topics that will lead to healthy pregnancy outcomes such as healthy eating, genetic testing, and family planning
  • Manage assigned patients, closely monitoring attendance at appointments, status of referrals, identifying and addressing barriers to care, and providing support and resources as needed
  • Manage activity of Individual Care Plan (ICP), assessing the effectiveness of interventions during each trimester and modifying as patient conditions change
  • Conduct prenatal orientation (intake) with patients, explaining procedures, answering questions, ordering labs, collecting medical/genetic history, reviewing pregnancy plans and assigning OB Clinician
  • Complete comprehensive health, nutrition and psychosocial assessment developing personalized ICP based on results and patient needs
  • Promote the prenatal program to clinic staff and patients in a positive, patient-centered and caring way
  • Proactively identify solutions to improve patient satisfaction, workflow and ways to achieve productivity goals
  • Refer clients to appropriate perinatal support services including nutritionist, social worker, childbirth educator, lactation consultant, WIC and dental
  • Coordinate referrals to imaging testing, specialists and delivering hospital including hospital pre-registration and hospital tour
  • Counsel patient for eligibility programs including Presumptive Eligibility, pregnancy Medi-Cal and Prenatal Cash Program
  • Collect specimen for pregnancy testing and generate Verification of Pregnancy
  • Follow established CPSP protocols and VCC guidelines
  • Document accurately in electronic health record and tracking logs
  • Meet or exceed established productivity standards and other metrics
  • Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings and workshops
  • Support the vision, mission and goals, and demonstrate a commitment to the values, of the organization
  • Perform other duties as directed



  • Bachelor’s degree
  • Minimum one year experience in perinatal, OB or gynecology care with a basic knowledge of how perinatal care is provided
  • Bilingual English/Spanish
  • CPR Certification (BLS Child, Infant, Adult)
  • Valid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes

Preferred Qualification

  • Medical Assistant certificate
  • Minimum three years’ maternal and child health experience, including direct patient services

Required training – must be completed within 90 days of hire

  • Presumptive eligibility online training (PE4PW)
  • Comprehensive Perinatal Services Program Provider Overview Online Training
  • Review the most current CPSP provider handbook, steps to take manual and the office CPSP protocols
  • Certified Family Planning Health Worker training

Required Skills/Knowledge/Abilities

  • Experience in case coordination or case management
  • Experience in assessing patients’ needs and providing direct health education in one-on-one or group sessions
  • Experience with a Medi-Cal program in an outpatient setting
  • Good multi-tasking skills with the ability to work in a fast-paced, high volume clinic environment
  • Ability to assess situations and identify solutions in an intellectually disciplined process
  • Excellent customer service skills and commitment to providing the highest level of customer satisfaction
  • Able to present information, both verbally and in writing, to individuals and groups
  • Ability to interface with all levels of personnel in a professional manner
  • Ability to work with people of all social and ethnic backgrounds
  • Ability to handle confidential materials and information in a professional manner
  • Good verbal and written skills necessary for communication with patients/clients, providers and other staff
  • Ability and willingness to meet the organization’s attendance and dress code policies
  • Experience/familiarity with computers, Microsoft Office products (Word/Excel), business e-mail, communication systems and internet search capabilities
  • Familiar with operation of telephone, fax and copier equipment
