Responsibilities

Job Summary

As a member of the pediatric care team, provide HealthySteps early childhood development intervention, referral and follow up, based on a risk stratified service delivery model.

Essential Job Functions

  • Conduct team-based well child visits in coordination with the primary care clinician visits focusing on monitoring development, social-emotional functioning and relationship health, and providing anticipatory guidance
  • Monitor and process calls from the child development support line
  • Attend reflective supervision meetings
  • Accompany families to key medical, specialty and community agency appointments as needed
  • Provide referrals and track follow-up as appropriate, to help families make successful connections to key resources
  • Participate in community-wide early childhood and/or mental health meetings as appropriate
  • May facilitate parent/caregiver groups
  • Work with front office staff to ensure timely scheduling of appointments
  • Work closely with pediatric primary clinicians around care coordination, goal setting, coaching and education about key aspects of a child’s development
  • Maintain databases required to meet HealthySteps’ fidelity metrics
  • Document patient clinical activity and care coordination in EHR
  • Track caseload to ensure capacity to deliver HS services within the risk stratified services delivery model including ensuring exit criteria is met and families and children are elevated to higher levels of care of referred to community providers as needed
  • Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings and workshops
  • Support the vision, mission and goals, and demonstrate a commitment to the values, of the organization
  • Perform other duties as directed

Qualifications

Minimum Qualifications

  • Bachelor’s degree in psychology, social work, counseling, early childhood education, or related field
  • Experience as a Licensed Clinical Psychologist, Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, or Licensed Professional Clinical Counselor
  • Experience and knowledge about early childhood growth and development, parent-child relational health, infant and early childhood mental health, and family systems
  • Valid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years’ driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record

  • Ability to collect and enter data for program management, evaluation and reporting purposes
  • Ability to present information effectively both orally and in writing to individuals and groups from various cultural, ethnic and educational backgrounds
  • Ability to interact with clients and personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs
  • Ability to multi-task and triage/juggle multiple appointments and conflicting priorities
  • Experience/familiarity with computers, business e-mail, communication systems, data entry skills and internet search capabilities; basic knowledge in Microsoft Office products, specifically Word and Excel

Pay Range: $35.00 - $49.00/hour DOE
